FOI and Publication Scheme
This is the Publication Scheme for Cygnet Orthodontics. The Freedom of Information Act 2000 (FOIA) requires all public authorities to adopt and maintain a publication scheme. The Freedom of Information Act 2000 aims to promote a culture of openness and accountability amongst public authorities by providing people with rights of access to information held by them.
This Publication Scheme is a complete guide to the information routinely made available to the public by Cygnet Orthodontics. It is a description of the information about our orthodontic practice which we make publicly available. Some information is not made publicly available. It is reviewed at least every two years and we monitor its effectiveness.
How much does it cost?
The publications are free unless otherwise indicated. Where information is provided at a cost the charges will be justifiable, clear and kept to a minimum. Charges may be made for activities such as printing, photocopying and postage. Anyone requesting information will be informed of any charges before the information is provided. We may ask for payment before we provide the information.
How is the information made available?
The information within each Class is either displayed on the practice website (www.cygnetorthodontics.co.uk) or available in hard copy from Cygnet Orthodontics, The Malt House, Wraysbury Road, Staines-Upon-Thames, Middlesex TW18 4XP.
Your rights to information
- In addition to accessing the information identified in the Publication Scheme, you are entitled to request information about Cygnet Orthodontics.
- The Freedom of Information Act 2000 recognises that members of the public have the right to know how public services are organised and run, how much they cost and how the decisions are made.
- It obliges Cygnet Orthodontics to respond to requests about information that it holds, and is recorded in any format and it will create a right of access to that information. These rights are subject to some exemptions which have to be taken into consideration before deciding what information it can release.

Classes of information
All information at Cygnet Orthodontics is held, retained and destroyed in accordance with current guidance. Our commitment to publish information excludes any information which can be legitimately withheld under the exemptions set out in the Freedom of Information Act 2000. Where individual Classes are subject to exemptions, the main reasons are e.g. the protection of commercial interests and personal information under the Data Protection Act 1998. Total annual expenditure on the provision of our contracted services – The practice provides both NHS and private treatment to patients. (Reasonable copying costs will be charged)
Audit of NHS Income – The practice is not subject to formal audits, so this information is not held.

Records of decisions made in the practice affecting the provision of NHS services – As a small business we do not normally maintain formal records of management decisions. Information may be available from Trevor Wynne-Hughes. However, any changes in the provision of NHS services would be incorporated and updated in our patient information leaflet. Our current patient information leaflet can be obtained from the practice reception. We may also notify patients by notices in our reception area.

Cost of Information
Most information is available for free, but for some there may be a charge – we will charge you only for hard copies or copying onto media (e.g. CD ROM) where necessary.
Charges are as follows:
Via the practice website www.cygnetorthodontics.co.uk information is free, although any charges for personal printing costs need to be met by the individual. For those without Internet access, a single print-out as on the website would be available by personal application at Cygnet Orthodontics, The Malt House, Wraysbury Road, Staines-Upon-Thames, Middlesex TW18 4XP. Requests for multiple printouts, or for archived copies of documents which are no longer accessible or available on the website may attract a charge for the retrieval of documents, photocopying, postage etc. We will let you know the cost and charges that will have to be paid in advance.
Leaflets and brochures – there will be no charge for leaflets or brochures that we provide at the practice as long as these are not requested in unreasonable quantities e.g. reasonable is deemed one copy per leaflet requested per person requesting the information
“Glossy” or other bound paper copies, or in some cases a CD Rom, video or other mediums may be charged for
E-mails will be free of charge unless otherwise stated
